Grant Reports

We look forward to hearing about the impact you are making in your community. The report you submit below is an important way to evaluate the quality of our programs and to promote the work you are doing.

  1. Download the appropriate Sample Report for you to prepare your answers and to gather any additional items, like pictures or links to videos.
  2. Download and Complete the Outcomes Form and Budget Form (Annual and Final Reports ONLY). Upload these documents when you submit the report online.
  3. Email 3-4 Pictures to grant.reports@wheatridge.org. Each picture should be .jpg files of around 500 KB each.
  4. Complete the Online Report Form. Review the questions in advance and plan to complete the form at one time. If you do not finish, be sure to save the report so you can revisit it at a later time.

To save answers and resume filling out your report later:

  • After clicking “Save Answers and Resume Later,” you will receive a link.  You must use this link to access your saved answers.
  • Please copy and save the link. It is the ONLY way to resume your form. You may also have the link emailed to you.
  • Unfortunately, we will not be able to get you that link again if it is lost. Without the link, the data you have entered cannot be retrieved and you will have to start filling out the form at the beginning.
  • You can use the link from any computer anytime within 30 days to resume answering questions on the form.

An email with a copy of your submitted report will be sent to the Program Contact. If you have any questions, please feel free to contact us at grants@wheatridge.org.

DO NOT send the report via email unless requested.
