Congregation Health Ministry Grants

The Congregation Health Ministry (CHM) Grant Program enables ministry leaders to move inspired whole person health and wellness ideas into action within congregations. These one-year, $2,000 matching grants for new and innovative programs are open to Lutheran congregations committed to becoming centers of health and hope in their community.

Projects including an anti-violence component will receive priority consideration in 2013 – click here for more information.
Congregations interested in applying for a grant must complete and submit the following items to grant.apps@wheatridge.org:
- Cover Sheet (attach as a .doc not a .pdf)
- Budget Form
- Proposal Guidelines
- Outcomes Measurement Plan
- IRS 501(c)(3) Confirmation Letter (Organizations located outside of the United States must provide evidence of equivalent corporate or trust status.)
- Letter of support from pastor/clergy or congregation president/chairperson (additional letters from other partners will be accepted)
- Organization’s most recent audited financial report or annual report
Application deadline is April 16, 2013. Awards will be announced by June 15.
Proposals will be reviewed by Wheat Ridge staff and a committee of ministry leaders based on our Grant Review Criteria.

Grant Information Webinar
This webinar was led by former Director of Impact Measurement and Grant Services, Kim Loontjer. All of the information in the webinar is still applicable to the current year of the Congregation Health Ministry grant program, with the exception that priority consideration will be given to projects with an anti-violence component in 2013.
